While claims for Term 4 open on Thursday December 18, please be aware that due to Christmas shutdown, claims cannot be processed and paid until the new year.

Conveyance Claim Form

I confirm that:
  • My circumstances have not changed since I was approved for a Conveyance Allowance. (Parents/carers must submit a new Application for Transport Assistance if the student’s circumstances change, e.g. if they change address or school.)
  • The information provided in my Conveyance Claim Form is complete and accurate.
  • I have included an official school record of the student’s attendance as proof of travel.
  • I have submitted my bank account details.
    • I understand that:
  • Conveyance Allowance claims are auditable. Fraudulent claims will be treated seriously, and the PTA will recoup any overpaid monies.
  • My entitlement to a Conveyance Allowance may be withdrawn if the information provided is false or misleading.
